Overview
Turn a description like "an incident report with injury details only if someone was hurt" into a working draft form.
It shows you what it plans to change and waits for a clear yes before saving anything. It works through the Imploy MCP connection, so it only sees what your Imploy role can access.
Free to download and use.
What it does
- Asks who fills the form in, what it's for and which sections it needs.
- Drafts the steps, fields, conditional questions and routing for you to check.
- Can set the form up to create incidents or prospects, or pre-fill client details.
- Leaves the form as a draft. It only publishes if you ask.
Example prompts
- Build an incident report form that asks for injury details only if someone was hurt, and creates an incident.
- Make a new client intake form that pre-fills the client's name, DOB and NDIS number.
- Create a 3-step client feedback form with a rating and a 'contact me' option.
How to install
- Connect the Imploy MCP server to your AI app and sign in to your Imploy organisation. Ask your assistant to run whoami to check it can see the right organisation.
- Download imploy-form-builder.zip.
- Claude: go to Settings, then Capabilities, turn on Skills and upload the zip. In a Claude Project, paste the contents of SKILL.md into the project instructions instead. In Claude Code, copy the folder into ~/.claude/skills/.
- ChatGPT: in a custom GPT or a Project, paste the contents of SKILL.md into the instructions and connect the Imploy MCP server.
- Other MCP clients such as Cursor or Windsurf: add the contents of SKILL.md to your custom instructions or rules file.
